MDT has established this Privacy Policy so that you can understand the care with which we intend to treat your personally identifiable information ("Personal Information") you share with us.
Personal Information means any information that may be used to identify an individual, including, but not limited to, a first and last name, a home or other physical address and an email address or other contact information, whether at work or at home.
We take your privacy concerns seriously and we strive to earn and keep your trust. Online privacy is a constantly evolving landscape and MDT plans to take reasonable efforts to maintain up-to-date and effective online privacy standards and practices. MDT has contracted with a third-party service provider to help track and analyze usage and volume statistics on this website. We use the Personal Information we collect to help both of us! As we mentioned above, registering with us allows you to personalize our website so that it is most useful to you. It also allows you to log into our website automatically each time you visit, rather than manually typing your user name and password every time. We may also use this information to periodically contact you with news or important information from MDT or to request your feedback on our website. 5.1 Special Offers and Updates
We send all new members a welcoming email to verify password and username. Established members will occasionally receive information on products, services, special deals, and a newsletter. 5.2 Newsletters
If you have signed up for our newsletters we may use your Personal Information to send you notifications about special offers or to tell you about opportunities available from our partners. 5.3 Service Announcements
On rare occasions it is necessary to send out a strictly service related announcement. For instance, if our service is temporarily suspended for maintenance or there are changes to our services we might send users an email. We communicate with users on a regular basis to provide requested services and in regards to issues relating to their account via email.
5.6 Tell-A-Friend
If a user elects to use our referral service for informing a friend about our website, we ask them for the friend’s name and email address. MDT will automatically send the friend a one-time email inviting them to visit the website. MDT stores this information for the sole purpose of sending this one-time email and tracking the success of our referral program. The friend may contact MDT at mail@MyDentalTrip.com to request the removal of this information from our database.
6. Choice/Opt-out
Our users are given the opportunity to ‘opt-out’ of having their information used for purposes not directly related to the Service at the point where we ask for information.
Users who no longer wish to receive our newsletter and promotional communications may opt-out of receiving these communications by following the instructions that appear at the bottom of the e-mail message in question or email us at mail@MyDentalTrip.com and placing the newsletter name and your username in the subject of the e-mail.
You can cancel your registration with us by e-mailing mail@MyDentalTrip.com, and placing your email address in the subject of the e-mail. We will send you an e-mail to confirm your request. After we receive confirmation, we will remove your information from our active database, but some information may remain in our archives.
6.1 HIPAA Deidentification Disclaimer – Your Obligation to Remove Identifiers.
The website, MyDentalTrip.com (“MDT”) and its owners, IDDMC, LLC, (“IDDMC”) are cognizant of the importance of protecting your health information and your privacy. Any person, member or user of MDT that provides health information or uploads personal health information, X-rays or other data (“Data”) to MDT does so voluntarily and does so with the knowledge that MDT and IDDMC are not health care providers, nor are they a “covered entity” under HIPAA, nor a “business associate” under HIPAA. Notwithstanding the above, you are encouraged to “deidentify” your Data to the greatest extent possible prior to uploading the Data to MDT. To that end, please remove all the following identifiers of the individual patient, its relatives, employers and household members; including names, geographic subdivisions smaller than a state, including removal of address, city, county, precinct, zip code and equivalent geocodes except for the initial three digits of a zip code (or simply remove the zip code completely), and all elements of dates (except year) for dates directly related to an individual including his birth date, admission date, discharge date, date of death and all elements of dates (including year) indicative of the age of an individual, except that age as an element that may be aggregated into a single category of age 90 or older; moreover, please eliminate telephone numbers, fax numbers, e-mail addresses, social security numbers, medical record numbers, health plan beneficiary numbers, account numbers, license numbers, vehicle identifiers including VIN, serial numbers and license plates numbers, device identifiers and serial numbers, web universal resource locaters (“URLs”), internet protocol (“IP”) addresses, biometric identifiers including finger and voice prints, full face photographs and comparable images and any other unique identifying number, characteristic or code.
Your ability to remove your confidential health information prior to providing the information and Data to our website will help us and you protect your health information to the extent possible. The Data you provide will be used as set forth in these Terms and Conditions
