Our website provides users with the ability to receive bids for dental work from our network of dentists (the “Services”). Your privacy on the internet is important to us.  As part of the operations of the Service, we gather certain types of information about the users. This Privacy Policy explains the types of information we gather, what we do with it and how you may correct or change information that we collect. By visiting our website and/or registering for the Services, you are acknowledging and accepting the practices described herein. MDT has established this Privacy Policy so that you can understand the care with which we intend to treat your personally identifiable information ("Personal Information") you share with us.
Personal Information means any information that may be used to identify an individual, including, but not limited to, a first and last name, a home or other physical address and an email address or other contact information, whether at work or at home.
We take your privacy concerns seriously and we strive to earn and keep your trust. Online privacy is a constantly evolving landscape and MDT plans to take reasonable efforts to maintain up-to-date and effective online privacy standards and practices. We have linked every page of our website to this Privacy Policy for your convenience. Our website is hosted in the United States of America and is subject to U.S. state and federal law. If you are accessing our website from other jurisdictions, please be advised that you are transferring your Personal Information to us in the United States, and by using our website, you consent to that transfer and to abide by the applicable laws of applicable states and U.S. federal law concerning your use of the website and your agreements with us. 3. Information We Collect

MDT has contracted with a third-party service provider to help track and analyze usage and volume statistics on this website. The third-party service provider does not collect Personal Information from website visitors, only general summaries of website activities. This information is used only to improve the website’s quality. This third-party service uses cookies and other tracking technologies (as explained below), neither of which contain personally identifiable information, to help in tracking and analyzing website usage information.

For example, you may post your own reviews, photographs and travel experiences and participate in our discussion forums. In addition, you will benefit from the ability to personalize your MDT website experience. The benefits of registering will increase over time as we get to know you better and introduce new features. We encourage you to register so that you can experience everything that MDT has to offer!

If you register with us, we will collect Personal Information from you in addition to the non-Personal Information described above. That Personal Information may include your name, email address, mailing address, telephone number, dental records and history, travel preferences, user name and password. The information we collect may vary, but we only collect the information that you manually enter into our forms or otherwise upload. We may store all or some of that information in a cookie file on your hard drive and on our servers, so that our system will recognize you each time you visit our website. In that way, we can save your preferences from visit to visit and present you with a customized website, without requiring you to log into our website every time you visit.

4. How we use the Personal Information we collect.

We do not provide to third parties any Personal Information you provide when using MDT except as set forth in this Privacy Policy or as otherwise notified to you. MDT is the sole owner of the information collected on this website.

We use the Personal Information we collect to help both of us! As we mentioned above, registering with us allows you to personalize our website so that it is most useful to you. It also allows you to log into our website automatically each time you visit, rather than manually typing your user name and password every time. We may also use this information to periodically contact you with news or important information from MDT or to request your feedback on our website. In addition to these periodic updates, we may email you additional materials, but only if you specifically request them. These might include opt-in newsletters and other materials that you proactively request from MDT.

We may also use certain information that you provide in order to present you with advertising that you may find interesting. This benefits both of us. It benefits us because advertising helps pay the bills, and we can charge our advertisers more if they know that their ads will be seen by somebody more likely to find it useful. • Dental Services Providers. When you request from us to receive bids on dental services, you are expressly authorizing us to transfer your Personal Information, including dental records and history, to members of our dental services network, and you are authorizing such members of our dental services network to contact you with information and pricing on their dental services. Additionally, if you are a member of our dental services network, you are expressly authorizing us to transfer your Personal Information to any of our users requesting dental services to be performed in the geographic area where your services are generally provided.
• Travel Services. When you request information on travel services, you are expressly authorizing us to transfer your Personal Information to members of our travel services network, and you are authorizing such member of our travel services network to contact you with information and pricing.
• By Law or to Protect Rights. When we believe disclosure is appropriate in connection with efforts to investigate, prevent, or take other action regarding illegal activity, suspected fraud or other wrongdoing; to protect and defend the rights, property or safety of our company, our users, our employees, or others; to comply with applicable law or cooperate with law enforcement; or to enforce our Terms and Conditions of Use or other agreements or policies, in response to a subpoena or similar investigative demand, a court order, or a request for cooperation from a law enforcement or other government agency; to establish or exercise our legal rights; to defend against legal claims; or as otherwise required by law. • Third Party Service Providers. We may share your Personal Information with our authorized service providers that perform certain services on our behalf. These services may include fulfilling orders, performing services offered through our website, providing customer service and marketing assistance, performing business and sales analysis, supporting our website functionality, and supporting contests, sweepstakes, surveys and other features offered through our website. 5. Communications from the Website

We do not send unsolicited e-mail. If you are receiving e-mail from us, it is because at some point you, or someone with access to your e-mail account, requested or consent to receive it. You can always opt out of any e-mail we send you simply by following the instructions that appear at the bottom of the e-mail message in question. If you are receiving e-mail from us in response to an inquiry you made, you can rest assured that we will use your e-mail address for no purpose other than to respond to your inquiry.

5.1 Special Offers and Updates

We send all new members a welcoming email to verify password and username. Established members will occasionally receive information on products, services, special deals, and a newsletter. Out of respect for the privacy of our users we present the option to not receive these types of communications. Please see the Choice/Opt-out sections.

5.2 Newsletters

If you have signed up for our newsletters we may use your Personal Information to send you notifications about special offers or to tell you about opportunities available from our partners. Although we do not want you to miss any great travel deals, you can always opt-out of any newsletter we send. Please see the Choice/Opt-out sections.

5.3 Service Announcements

On rare occasions it is necessary to send out a strictly service related announcement. For instance, if our service is temporarily suspended for maintenance or there are changes to our services we might send users an email. Generally, users may not opt-out of these communications, though they can deactivate their account. However, these communications are not promotional in nature.

5.4 Customer Service

We communicate with users on a regular basis to provide requested services and in regards to issues relating to their account via email. 

5.6 Tell-A-Friend

If a user elects to use our referral service for informing a friend about our website, we ask them for the friend’s name and email address. MDT will automatically send the friend a one-time email inviting them to visit the website. MDT stores this information for the sole purpose of sending this one-time email and tracking the success of our referral program. The friend may contact MDT at mail@MyDentalTrip.com to request the removal of this information from our database.

6. Choice/Opt-out

Our users are given the opportunity to ‘opt-out’ of having their information used for purposes not directly related to the Service at the point where we ask for information.

Users who no longer wish to receive our newsletter and promotional communications may opt-out of receiving these communications by following the instructions that appear at the bottom of the e-mail message in question or email us at mail@MyDentalTrip.com and placing the newsletter name and your username in the subject of the e-mail.

You can cancel your registration with us by e-mailing mail@MyDentalTrip.com, and placing your email address in the subject of the e-mail. We will send you an e-mail to confirm your request. After we receive confirmation, we will remove your information from our active database, but some information may remain in our archives.

 

6.1 HIPAA Deidentification Disclaimer – Your Obligation to Remove Identifiers.

 

The website, MyDentalTrip.com (“MDT”) and its owners, IDDMC, LLC, (“IDDMC”) are cognizant of the importance of protecting your health information and your privacy. Any person, member or user of MDT that provides health information or uploads personal health information, X-rays or other data (“Data”) to MDT does so voluntarily and does so with the knowledge that MDT and IDDMC are not health care providers, nor are they a “covered entity” under HIPAA, nor a “business associate” under HIPAA. Notwithstanding the above, you are encouraged to “deidentify” your Data to the greatest extent possible prior to uploading the Data to MDT. To that end, please remove all the following identifiers of the individual patient, its relatives, employers and household members; including names, geographic subdivisions smaller than a state, including removal of address, city, county, precinct, zip code and equivalent geocodes except for the initial three digits of a zip code (or simply remove the zip code completely), and all elements of dates (except year) for dates directly related to an individual including his birth date, admission date, discharge date, date of death and all elements of dates (including year) indicative of the age of an individual, except that age as an element that may be aggregated into a single category of age 90 or older; moreover, please eliminate telephone numbers, fax numbers, e-mail addresses, social security numbers, medical record numbers, health plan beneficiary numbers, account numbers, license numbers, vehicle identifiers including VIN, serial numbers and license plates numbers, device identifiers and serial numbers, web universal resource locaters (“URLs”), internet protocol (“IP”) addresses, biometric identifiers including finger and voice prints, full face photographs and comparable images and any other unique identifying number, characteristic or code.

 

 

Your ability to remove your confidential health information prior to providing the information and Data to our website will help us and you protect your health information to the extent possible. The Data you provide will be used as set forth in these Terms and Conditions

 

7. Collection And Use Of Information By Third Parties Generally

Company contractually prohibits its contractors, affiliates, vendors and suppliers from disclosing Personal Information received from us, other than in accordance with this Privacy Policy. MDT makes reasonable efforts to maintain the security of your account information, and has reasonable security measures in place to protect the loss, misuse and alteration of the information stored in our system. For example, we limit access to Personal Information about you to employees who we believe reasonably need to come into contact with that information. We also employ processes (such as password hashing, login auditing, and idle session termination) to protect against unauthorized access to your Personal Information. However, information transmitted over the internet is never 100% secure and there is no way for us to guarantee the security of your Personal Information.

The Website encrypts sensitive information such as your credit card number and your Personal Information using secure socket layer (SSL) technology to provide for the secure transmission of the information from your computer to our servers. 9. Children's Privacy

This website is a general audience website, and we do not knowingly collect information about children. California Civil Code Section 1798.83, also known as the "Shine The Light" law, permits our users

a list of the categories of Personal Information that was shared and the names and addresses of
